We have an exciting opportunity for  Key Worker to join our Targeted Care Packages Program, based between Uniting Wesley in Dandenong and Uniting Connections in Pakenham and Narre Warren offices.

 

The Targeted Care Packages program:

  • The purpose of the Targeted Care Packages is to enable the transition of eligible children and young people from residential care to more appropriate care arrangements where their care needs will be better met. Alternative placements which could be considered as part of the package include placements with parents, family or friends, a foster carer, independent or semi-independent living or other forms of care.
  • The packages include an allocation of funding tailored to the assessed needs of a child. The initial priority groups of the packages are children aged 12 and under and Aboriginal children and young people

The Key Worker – Targeted Care Packages will

  • Provide direct service delivery for families to promote family preservation where children and young people are at imminent risk of entering residential care; or case management and direct service delivery to support children and young people to exit residential care to a more appropriate placement, typically foster care or kinship care.
  • Assist families directly in their home and community to build skills, improve confidence, establish routines and increase the overall safety in the home.  They will also support attendance at services and engagement in activities and promote education attendance and participation.
  • Complete observational parenting assessments to inform the completion of an outcomes measure tool and regular, formal case reporting.  These assessments inform a more objective approach to goal setting and also reinforce the child, young person and parents’ progress in order to build their confidence.
  • Be directly supervised by the Coordinator of TCP and Specialised Home Based Care, Wesley with secondary supervision from a Team Leader or Program Leader, Placement Prevention and Reunification, Connections.

  • Describe your experience developing, implementing, monitoring and reviewing high quality, trauma informed and tailored plans to support children, young people and families to promote placement preservation or support children and young people's transition to a more appropriate placement type.
